New Report of Check Exchange Form for OSC's Bureau of State Payroll Services


To notify agencies of the new Report of Check Exchange form (AC 1476-P) for payroll transactions.

OSC Actions

The Report of Check Exchange form (AC 1476-P) has been revised to accommodate additional required information.

Agency Actions

Agencies must provide all required Social Security or Taxpayer Identification numbers for the estate of a deceased employee, along with the address(es) of the recipient(s) which will be used for mailing Form 1099-MISC if applicable.

This form (AC 1476-P) is to be used for payroll transactions only.
