Help for Completing the Student Achievement Award Nomination Form

 

Nominate a student for the New York State Comptroller's Student Achievement Award online. Fields that are marked with an asterisk (*) are required.

 
Student Name* Enter the full name of the student you’re nominating exactly as it should appear on the award certificate. (Do not enter all capital letters.)
School Name* Enter the name of the school the nominated student will be graduating from. While the legal name of the school isn’t necessary, please be consistent across nominations.
PIN* Enter the 4-digit Personal Identification Number (PIN) provided to you in the letter you received from the Office of the State Comptroller. Without a PIN, your nomination can’t be processed.
First Name* Enter the first name of the person to whom the award should be mailed.
Last Name* Enter the last name of the person to whom the award should be mailed.
Email Address* Enter the email address of the person requesting the award.
Phone Number Enter the 10-digit telephone number of the person requesting the award.

City, State*
Enter the address where the award certificate will be mailed.
ZIP Code* Enter the 5-digit ZIP code where the award certificate will be mailed.
Presentation Date* Enter the date of the student’s graduation/ceremony.
Event Location Enter the location of the event.

City, State
Enter the address of event location.
ZIP Code Enter the 5-digit ZIP code for the event location. Enter numbers only in this field and do not include the plus4 numbers that may appear after the ZIP code.
Notes Enter any additional information about the graduation/ceremony, i.e., time of event. If you have special requests or instructions, list them in this field.

Multiple Nominations
After you submit a nomination, you’ll be taken to a Confirmation page. If you want to nominate another student, you can use the link on that page to create a new nomination. You will need to complete all the information again.

Next Steps
Once we receive your nomination(s), we will send you a confirmation email within two business days. After we review the information, we’ll mail you the award within two weeks.
