Purpose
To inform agencies of the changes to the following forms:
TIAA-CREF Action Request (AC 1767) and Application For Refund of Employee TIAA-CREF Contributions (AC 1781).
Background
To comply with personal privacy laws, both forms have been revised and require employees to provide only the last four (4) digits of their Social Security number (SSN).

Employees who enroll or withdraw membership in TIAA-CREF

OSC Actions
OSC will now require employees to supply only the last four (4) digits of the SSN on the TIAA-CREF Action Request (AC 1767) and Application For Refund of Employee TIAA-CREF Contributions (AC 1781) forms.
Agency Actions
Agencies should destroy all previous Application For Refund of Employee TIAA-CREF Contributions forms (AC 1781). Both revised TIAA-CREF Action Request (AC 1767) and Application For Refund of Employee TIAA-CREF Contributions (AC 1781) are available on the Payroll pages of the OSC web site under Information for Payroll Officers.
Note: The previous AC 1781 was a three part form. The revised form is one page. Agencies are advised to make a copy of the completed form before sending to OSC. OSC will no longer mail agencies a copy of the AC 1781 when the refund is processed. Agencies can view the refund information on the retirement plans table when TIAA-CREF refunds are processed.
